This form releases the Prescott Vette Sette, LLC and the Prescott Vette Sette Charitable Foundation, Inc., hereinafter referred to as the “Club”, and the Club Board Members and Officers, from any and all liability that may arise as a result of my participation in any and all Club activities, events and runs, hereinafter referred to as “Activities”. 

I am voluntarily participating in the Club Activities, and I am participating at my own risk. I am aware of the risks associated with traveling to and from, as well as participating in, the Activities, which may include, but are not limited to, physical, psychological injury, pain, suffering and illness, disfigurement, temporary or permanent disability, economic or emotional loss, and death. I understand that these injuries or outcomes may arise from my own or others’ negligence, conditions related to travel, or the condition of the activity location(s). Nonetheless, I assume all related risks, both known and unknown to me, arising out of or resulting from my participation in such Activities.

I hereby agree to waive and forever release the Club, and its Board Members and Officers, against all liability for personal injury, damages including damages from physical or psychological injury, pain, suffering and illness, disfigurement, temporary or permanent disability, economic or emotional loss, and death arising from or related to my participation in Club Activities. I further hereby indemnify and hold harmless the Club, its Board Members and Officers for any claims, suits or action of any kind whatsoever for personal injury, liability, damages, compensation, legal fees or otherwise brought by me, or anyone on my behalf, in connection with or arising from my participation in Club Activities and for any such suits or claims arising from, or in connection with, any family, friends, or any Guests that may be accompanying me during Club Activities. I agree that I will maintain adequate and appropriate automobile insurance coverage while participating in any Club Activities.
